In a manufacturer-supported, multicenter trial, investigators randomized patients age range, 6—17 years with ADHD to receive placebo or one of three doses of an extended-release form of guanfacine 2, 3, or 4 mg daily, starting at 1 mg daily and increasing by 1 mg weekly to the assigned dose. After 5 weeks, dosages were tapered during 3 weeks. At baseline, mean scores on a standard ADHD rating scale were similar in treatment and placebo groups. By week 3, mean changes in ADHD scores were significantly greater in the guanfacine groups, combined and individually, than in the placebo group, with the greatest benefit in the 4-mg—guanfacine group.
Similar improvements were noted on teacher, parent, and clinician global assessments. Specifically, guanfacine was associated with significant improvements in symptoms of hyperactivity, impulsivity, and inattentiveness, although the change from baseline in patients with the inattentive ADHD subtype was not statistically significant.
Younger children age range, 6—12 years had greater responses to guanfacine than did adolescents age range, 13—17 years. In a government-sponsored, double-blind, placebo-controlled study, investigators randomized children age range, 7—12 years with ADHD to one of four treatment groups: clonidine, methylphenidate, clonidine plus methylphenidate, or placebo.
At 16 weeks, compared with placebo, teacher assessments on a standardized ADHD symptom scale the primary efficacy measure did not show improvement in the clonidine-only group but did indicate statistically significant improvement in both methylphenidate groups. However, parents of children in the clonidine-only group, but not the methylphenidate-only group, reported significant symptom improvement. Sedating effects were reported more often with clonidine. Teachers, parents, and clinicians found that hyperactivity and impulsivity were reduced significantly in children with ADHD who were treated with extended-release guanfacine, compared with children who received placebo.
Although these results are encouraging, we need more studies to establish the use of guanfacine for treating core ADHD behaviors. Parents, but not teachers, reported improvements in ADHD symptom scores with clonidine. Off-label use of these medications is not encouraged, but, when a clinician decides to prescribe them, informed consent should include information about the off-label status.

My PCP recommended Clonidine. It really helps me relax and focus within about minutes. If I take 2 during the day, I get too sleepy. I do take 2 before bed and it helps me sleep really well. So far so good! My Med. So we tried Clonidine and I am so thankful we did.I take clonidine 0. I have not had much trouble with side effects, the first couple days it knocked me out, but within a week that had resolved. I have gained maybe 10 lbs in the past 3 years which I've taken it. I also take 25 mg HTCZ. Between the two my blood pressure has been under control. I'm also perimenopausal and suffer from migraines. I think the clonidine has also helped with these conditions.
I was finally getting off of suboxone and my dr prescribed Clonidine and I have no withdraw symptoms I shouldn't say all but the ones that make you feel that your skin is inside out. Prescribed 25mcg, one up to four times per day, started two months ago.

Clonidine has worked wonders with helping me get through my addiction. I have been clean from opiates but I take suboxine. I am currently tapering my dose of suboxine down, I am on a very small dose right now.
The clonidine is the only thing that helps with the RLS I get from withdrawling. The RLS is probably the one side effect that I cannot tolerate during opiate withdrawl because it makes it impossible for me to fall asleep.

I also take it as an additive med for hypertension. I currently take Diovan as well. So far, my pressure has gone down but my pulse has jumped over 90 at rest. I know that it takes time for meds to work especially for ADHD but for hypertension it helps immediately.
I'll contact my doctor and try breaking. Hopefully I'll still sleep well but I can't afford to be sedated all day plus coffee is something I must avoid as well since I am prone to elevated levels of anxiety.

The only side effect I get from Clonodine is a little tiredness, but that passes fairly quickly and its worth it to me to not have the bad feeling I get when my bp is high. So, I finally figured out if I take it every 8 hours now 6am, 2pm, 10pm it keeps my bp in a perfect range all day.
This drug also helps me to go to sleep at night and stay asleep whereas before I always had a bit of insomnia. I recommend it IF you are extremely regimented and good at taking medicine on time at the same time.
If you miss a dose of this medicine or don't take it at the same time you can get dangerous rebound hypertension.
